1. W9S

(BYU-Pathway Worldwide Online) Brigham Young University-Idaho Information
The W9S is a form that helps the University make sure we have the same information on the school system as the IRS. Your name and Social Security Number must match on both systems (for international students, it's a TIN number). If you do not, we cannot make a 1098-T. Students who need assistance with this process should contact BYU-I Academic Services.
  • You will need to fill out a W9S and attach it to the request for an unavailable 1098-T form:   Request an Unavailable 1098-T Form
  • You will be sent an email if their information doesn’t match with instructions on how to complete the W9S form. You then need to email the form to AccountingServices@byui.edu

The TL11A form (Tuition, Education, and Textbook Amounts Certificate – University Outside Canada) in Canada is similar to the 1098-T. Canadian students need this in order to file taxes in Canada.
Canadian students will need to fill out this form ”REQUEST FORM” and Accounting Services will send the document to their BYU-Idaho email address. If a student is wanting to request multiple forms for more than one year, they will need to submit a form for each year they are requesting.
